IChemE - Process Safety – Five Decades of War Stories

Process safety is an integral part of most chemical engineers responsibilities. It has been a significant feature of the presenter’s career since his first role as a chemical engineer on a petrochemical site in the mid-1970s. All of the process safety case studies presented are cases that the presenter has been personally involved with or incidents that have left a life-long impression. The incidents will cover permit to work, management of change, design short-comings, operational changes, competency, piping and equipment failure, and hazard analysis. Since almost all chemical engineers are involved with process safety, the talk will be relevant to a large swathe of the chemical engineering community. Attendees will be provided with information to help with future risk identification and management.
